A 64-year-old woman was admitted to Legacy Pointe in Iowa City for diabetes management and medication administration. During her stay, staff was not administering medication as required. She also suffered several falls, although she was noted as a high fall risk. She became septic and severely dehydrated due to the mistreatment and eventually died of her injuries.
